Flu consent forms were given to students to take home on Monday, August 30. First District
Health will be at the school on October 19th to give flu shots. If you
would like your child to receive a shot, please complete the form and
return it to the school as soon as possible. Additional forms are
available in the school office or visit www.fdhu.org. Thank you!
